My experience has been that respir that does not respond to diff. combos of antibiotics is the sympton I notice.
But once rats are on the three or four heart meds, they no longer have serious respir problems as the meds manage the symptoms.
Usually the rats do not have seriously damanged lungs, perhaps this is because with experience, we are catching it earlier.
It is hard to know which of the two is the initial problem as untreated both affect one another and will make each other worse.

Good point and one I will use with my vet if the time comes when I must push for heart meds.
 
My vet says the same thing about heart meds Jo. If I really wanted to push to get them I probably could.
